A QMS (Quality Management System) is a repository which contains the Processes, Policies, Procedures and applicable artefacts like templates, checklists and guidelines.
At CMMI Maturity Level 3 there is a requirement of the Organizational Process Definition which requires that a common set of processes shall be available in the organization.

As defined by the CMMI in the CMMI Model:
The purpose of Organizational Process Definition (OPD) is to establish and maintain a usable set of organizational process assets, work environment standards, and rules and guidelines for teams.
CMMI Maturity Level has 18 Process Areas in total – 07 Process Areas from CMMI Maturity Level 2 and 11 Process Areas from CMMI Maturity Level 3. So while developing the QMS we have to take care to design the QMS in such a way that all applicable Process Areas shall be covered by it.
The structure of QMS can be designed in Layers as shown below:
Layer 1 – At the top level Categories (like Project Management, Process Management, Engineering and Support) can be there.
Layer 2 – At the Second Level each category can have processes as per the category. For e.g. Project Management processes shall come under Project Management category, CM process can come under Support category, etc.
Layer 3 – At this layer all associated artefacts like Templates, Forms, Checklists, Guidelines etc. can come under the processes.
